Eligibility: Homeless or at emerged risk of homelessness, diagnosed with serious mental illness and resides in Cameron, Clearfield, Elk, Jefferson, McKean or Potter Counties.
Targeted age group(s): 17 - 25

Resource management services are provided to assists adolescents/young adults diagnosed with mental illness that are homeless, making referrals for housing and support services.
